UNDERSTANDING WILLS AND TRUSTS

  • Contact information for person nominated to serve as personal representative (known elsewhere as an executor)
  • Contact information for person nominated to serve as the alternate personal representative
  • Contact information for person nominated to serve as second alternate personal representative
  • Contact information for beneficiaries (whether human or charities)
  • Dates of birth for any minor beneficiaries
  • List of assets, if any, that you wish to have distributed to specific beneficiaries
